For your convenience, our New Patient Forms are available for reviewing and filling out at your leisure, saving you valuable time at your first appointment. Simply download, print, complete the forms and bring them with you on the day of your visit. If you need to complete them at our office we have them at the front desk. We just ask you be a bit early so your appointment is on time.

  • During your first visit we will need to obtain important information, such as medical history, current dental status and any insurance information for billing purposes.
  • You will find more about our practice by exploring our website. You will find information such as maps, directions, office hours, payment policies and much more. 
  • There are bios about each of our staff members and a step by step description of what to expect at your routine preventive visit.
  • Your oral health and the appearance of your smile have a large impact on your life. A bright smile greatly increases self-confidence and makes a positive impression on the people you meet. The condition of your teeth and gums also affect aspects of your physical health. To maintain your oral health, it is important to develop good oral hygiene habits and schedule routine preventive visits.
  • Most preventive cleaning occur on a six-month schedule. Some patients may require a shorter re-care appointment if they have periodontal issues or struggle with tarter or stain build-up.
  • You can save time by printing and completing all the patient forms http://www.maggiekleemdds.com/new-patient-forms.html and bring them to your first appointment. If you are unable to print the forms they will be available at the front desk of your first visit. If you need to complete your paperwork at our office we ask if you can be 10 minutes early to complete the forms. We want your appointment to start on time.
